At the time of S.N.’s death, the family had an open Child Family Welfare Services (CFWS) case. The coroner’s report which stated S.N.’s cause of death was combined toxic effects of fentanyl and methamphetamine with a contributory condition of unsafe sleep environment (co-sleeping with an adult and placed on stomach) and the manner of death was undetermined.
